Due to the differences between Ubuntu and Centos around Apache2 I have
had to copy both start.sh and config-external.sh to be unique files
for Ubuntu so as not to affect Centos either. This can be removed when
we change the way config-external works.

Rabbitmq containers mount /var/lib/rabbitmq from 'data'
containers with 'root:root' permissions because
ansible does not wait for rabbitmq bootstrap container -
the latter corrects access rights but sometimes gets
killed just after start.

The previous method of using the API image for a data container
worked, but resulted in more data being bindmounted and copied
into the system. This patch resolves that by essentially using
a distro-type as a FROM in a Dockerfile to generate
distro-type-data. Further all of the data containers used
throughout ansible are modified to use this new data container.

The purpose for having defaults.yml on the command line was originally
for variable inheritance. Over the past month we have slowly changed
how things are inherited (like ports) allowing us to place these
'defaults' into the group_vars/all.yml location as they should be
values that go unchanged (but can be overridden).

When running cinder-ansible, the cinder db is never being initialized.
The bootstrap task needs to be fixed in order for cinder to run the db sync.

Every service except cinder-api was looking for the file
'/opt/kolla/cinder/cinder-<service>.conf' when the config file in place was named
'cinder.conf'. Change the rest of the services to look for cinder.conf or
the config file will never be copied over to into '/etc/cinder/'.

Glance bootstrap container is based on glance-api. Ansible playbook
incorrectly mounts
"{{ node_config_directory }}/glance" to "/opt/kolla/glance"
where it should mount
"{{ node_config_directory }}/glance-api" to "/opt/kolla/glance-api".
After start bootstrap container fails to find configuration and exits.

Bootstrapping of Keystone/Glance happens on first host
via 'run_once'. After running bootstrap containers we
should wait for them also on the same host. Currently
waiting is done on all hosts what gives task error on
other hosts.

For rabbitmq, those options will only be needed once ceilometer is
